The integrated course in Medical-Scientific Methodology and Translational Human Sciences and Medical Informatics
aims to provide students with the basic knowledge needed to prevent disease and
promote the health of individuals and the community in the second year. Students will study the fundamental concepts
of epidemiology and sociology, contextualized to the concept of health according to the bio-psycho-social theoretical model.
The course will provide knowledge of the proximal and distal determinants of health,
the main indicators (socio-health, demographic, health), the main causes of morbidity and mortality,
and risk factors. The basic concepts of medicine in the workplace will be covered. Part
of the course will be dedicated to illustrating the evolution of medical ethics and the concept of everyday ethics.
The essential principles of scientific communication methodology and "Evidence-Based Medicine" will be introduced, and the application of the essential principles of health economics in medical
decision-making will be discussed.
The medical informatics course aims to provide basic knowledge of the technological and methodological concepts
for the use of data and information, communication rules, and the possibilities for healthcare professionals to use health information systems and digital healthcare.

The course introduces the foundations of epidemiology through a historical examination of John Snow’s investigation of the cholera outbreak in London, which is used as an example of the application of the epidemiological method to the study of health and disease. The main demographic, health and social indicators are subsequently presented, with particular emphasis on the infant mortality rate, its significance as an indicator of a population’s health and level of development, and its historical evolution. Within this framework, the United Nations Millennium Development Goals and Sustainable Development Goals are also discussed.
The concepts of crude and specific rates are examined, together with the essential principles of demography. Changes in population structure and patterns of health and disease are analysed through the theories of demographic transition and epidemiological transition. The Framingham Heart Study is presented as a landmark investigation for the identification of cardiovascular risk factors and for understanding the multifactorial nature of chronic diseases. This approach is further explored through the example of cardiovascular risk charts.
The course covers the main measures of disease frequency, with particular reference to prevalence and incidence, as well as common measures of association, including relative risk and the odds ratio. The essential characteristics of the principal epidemiological study designs are also introduced, including descriptive, cross-sectional, case-control, longitudinal cohort and experimental studies. Particular attention is devoted to the concept of causality in epidemiology and to the criteria used to assess possible causal relationships between exposures and health outcomes.
The concept of health is examined through its main definitions and its historical and cultural development. Proximal and distal determinants of health are analysed, together with their role in the production of social and health inequalities. The principles of health promotion are discussed in relation to the historical and programmatic significance of the 1978 Alma-Ata Conference and the 1986 Ottawa Conference.
The natural history of infectious and chronic degenerative diseases provides the basis for the analysis of the different levels of prevention: primary, secondary and tertiary prevention. The course explores the role of health education, the aims and requirements of screening programmes and the major public health issues associated with antimicrobial resistance.
The global burden of disease is addressed through the concept of burden of disease, the analysis of mortality and the use of Disability-Adjusted Life Years, or DALYs. The main non-communicable chronic diseases affecting contemporary populations and their associated risk factors are subsequently examined.
A specific section of the course is devoted to smoking as a major risk factor for disease, for which no specific medical training is provided.
Epidemiological data concerning the use of conventional tobacco products, electronic cigarettes, heated tobacco products and other emerging nicotine products are analysed. The course addresses the mechanisms of nicotine dependence, the principal pathophysiological effects of tobacco and nicotine use and the health consequences of direct exposure, second-hand smoke and forms of exposure commonly referred to as third-hand and fourth-hand exposure. The environmental harm associated with the production, consumption and disposal of tobacco products is also examined.
The course concludes with the principles of medical support for tobacco and nicotine cessation, with particular reference to the health benefits of quitting, the stages of behavioural change, brief motivational advice and the main pharmacological treatment options. Introductory information is also provided on tobacco-control legislation and policies adopted at the Italian and international levels.
