Recognize limitations that could impact the achievement of the expected outcome within the given timeframe, such as: age, medications, socioeconomic status, comorbidities, cognitive status, social resources, nutritional status, and environmental context. Define the relevant therapeutic objectives and intervention priorities. Define and write realistic goals that consider all patient variables, through expected short-, medium-, and long-term changes (both through intrinsic recovery and the use of compensation or alternative strategies). Formulate goals according to SMART principles (Specific, Measurable, Acceptable, Realistic, Time-bound). Identify the duration and frequency of interventions in the rehabilitation project planning, including cost-effectiveness. Select and prioritize essential interventions that are risk-free and match the specific functional objectives and outcomes of the treatment plan. Identify precautions and contraindications. Ensure patient-centered interventions. Define the specifics of the intervention (time, intensity, duration, and frequency). Select realistic priorities that consider treatment time in collaboration with family, caregivers, and other professionals. Develop intervention strategies based on assessment data, the best available evidence, and patient choices/preferences.
