Part VII – Research Activities
Prof. C. Scagnolari coordinates and carries out the research activities of the Virology Laboratory (Department of Molecular Medicine), which are mainly dedicated to antiviral innate immunity and virus-host interaction. For years, Carolina Scagnolari's research has focused on the analysis of the activation of the interferon system (IFN type I, II and III) and its antiviral activity, a key component of innate immunity, during acute and/or chronic viral infections of public health interest. In this context, part of her past research has been devoted to the identification of factors associated with response to IFN therapy. In addition, she has helped to demonstrate that sustained and increased IFN production promotes the exacerbation of several viral infections, but at the same time, reduced expression of these antiviral systems due to viral and host factors can contribute to severe disease. C. Scagnolari's interdisciplinary collaboration with other national and international research groups has allowed the clinical significance of viral load to be assessed and the molecular epidemiology of emerging viruses to be characterised in detail. In recent years, C. Scagnolari's scientific interest has also focused on the analysis of the effect of oral bacterial supplementation in HIV-1 and HPV infections, with important findings on inflammation and T-cell immunity.

• Factors associated with response to interferon (IFN) therapy
Throughout her career, C. Scagnolari has been investigating the host factors that need to be considered to explain the heterogeneity of clinical response to IFN therapy in patients with various chronic diseases such as multiple sclerosis, chronic hepatitis C infection and cryoglobulinemia. These studies have helped to highlight the important role of anti-IFN neutralizing/binding autoantibodies, IFN-related genes/microRNAs and IFN gene variants (IL-28B; IFNlamda4) in IFN-mediated response and have provided insights into the host factors that influence the outcome of IFN treatment. • Emerging viruses and IFN resistance
Over the years, a major focus of C. Scagnolari's research has been the study of host-virus interactions at the cellular and molecular level, particularly in the context of viruses and IFNs. During the SARS-CoV-1 outbreak in 2003, C. Scagnolari was one of the first researchers to describe the resistance of SARS-CoV-1 to IFN action. Encouraging results have led her group to investigate IFN activity against a number of emerging viruses, including human metapneumovirus, pandemic H1N1 influenza virus, mosquito-borne viruses, SARS-CoV-2 and monkeypox virus. She has also made several major contributions to the antiviral characterisation of IFNalpha and IFNlambda subtypes. • IFN production in acute and chronic viral infections
In recent years, C. Scagnolari has spent much of her time in collaborative, multidisciplinary research settings, and through this experience it has become increasingly clear that although IFNs are universally recognised for their antiviral and immunostimulatory functions, they can also cause decreased host resistance to some viral infections. In this context, C. Scagnolari has made significant contributions to our understanding of the protective or deleterious effects of TLRs, type I/III IFNs and ISG responses in patients infected with HIV-1 and/or HPV, in patients suffering from dengue disease and in those with respiratory viral infections, including those caused by SARS-CoV-2. More recently, she found that patients with severe COVID-19 or coinfection with SARS-COV-2 and HIV develop anti-IFN neutralizing/binding autoantibodies and that an alteration in IFN response characterizes long COVID. In the last decades, C. Scagnolari, in collaboration with Prof. Raphael Viscidi (Johns Hopkins University, USA, Visiting professor in her laboratory), has also carried out research to study the IFN response in COVID-19 patients and to evaluate a recently discovered mouse papillomavirus (MmuPV1) as a model for the analysis of innate immune responses to papillomavirus infection. Although her research interests have largely focused on IFN responses to viral infections, C. Scagnolari has also contributed to important studies on the genetic diversity and molecular epidemiology of respiratory viruses, including respiratory syncytial virus (RSV) and SARS-CoV-2, HPV and polyomaviruses. She has also participated in studies on the relationships between RSV or HPV load, viral clearance dynamics and disease severity. C. Scagnolari has also investigated the ability of oral bacteriotherapy to modulate epithelial barrier functions, microbiota composition, neuroinflammation, IFN pathways, T cell subsets (e.g. Th17) and T cell immune activation in people living with HIV. More recently, she found that clearance of anal HPV infection was more frequently observed in MSM patients receiving oral bacteriotherapy supplementation and that a specific microbiota signature characterised SARS-COV-2 positive patients.
